I grant permission for Tactical K-9 LLC and its authorized agents to provide the services checked above. I will inform the business of any special medical, behavioral, or safety concerns in advance.
In an emergency I authorize staff to transport my dog to the nearest veterinary clinic and make necessary medical decisions if I cannot be reached. I agree to cover all veterinary expenses.
The business will exercise reasonable care. Dogs may still be injured or ill. Tactical K-9 LLC does not guarantee against those outcomes. After my dog is returned I accept full responsibility for its care, behavior, and safety, including behavior relapse, injury or illness after returning home, reactions to delivered food, and accidents after drop-off or transport.
Dog training and handling involve risk. I will not hold the business, staff, or affiliates liable for injury, damage, or loss during or as a result of training or care. Training is not guaranteed 100% effective. Tactical K-9 may discontinue services if continuing poses a safety risk. This agreement is governed by Florida law.
